something about the way this is illustrated is REALLY weird...the cigarette looks like it's sticking out of his tooth instead of being held between. it's messing with my head!

I totally get what you mean! I wonder if it's because the cigarettes don't have a coloured tip to mark where it begins. It's possible they couldn't actually get Ronald Reagan and pasted his head from somewhere on someone else's body. His shoulders seem a little hunched.
