I think the idea is that he's got the club tucked under his armpit and over his forearm much as one'd carry a shotgun. That seems like it'd be an awkward way to hold something as thin as a golf club, though, so maybe it explains why he's resting it on her hoochie instead.

well her right side had a huge shoulder deformity due to the time she snickered when he missed the putt on the 9nth hole and it put it 10 over par....her boobies are odd looking because of the time he got the ball stuck on the rough and lost the bet with his boss and had to buy everyone a round....

her neck is elongated because of the time they got lost and she suggested he ask for directions, oh 8 months in traction....

I think the joke is about the ad, rather than domestic violence. I see something similar in it - the ludicrous adoring expression she's been drawn with, her attitude of deference.
